Employers with dental benefits through Guardian Dental should call Guardian’s Customer Service Team at 800-627-4200 with questions and comments.

SBCs are mandated under the Patient Protection and Affordable Care Act (PPACA) and the SBCs provided by SummaCare will reflect medical and pharmacy benefits only. They will not reflect or include details about Health Reimbursement Accounts (HRAs), Health Savings Accounts (HSAs) or other ancillary benefits. SBCs for tailored benefit plans are available upon request from your Account Manager.
Employers who offer prescription drug coverage through SummaCare can use our drug search, find copay information or an in-network pharmacy and learn more about SummaCare’s mail order pharmacy service.
