Case Management Services

SummaCare has Registered Nurse case managers available to provide education to members on self-management of disease conditions and to assist in coordination of services.

Members who have received care coordination through SummaCare’s Case Management program had a 1.4 percent decrease in Future Inpatient Risk trend*, while members who did not participate in SummaCare’s Case Management program had a 1.3 percent increase in Future Inpatient Risk trend.

*Future Inpatient Risk refers to the probability of future hospitalization over the next 12 months. The average risk for the general population is 3 percent.
