SummaCare’s Disease Management programs provide valuable tools and resources to help our members manage chronic conditions and live a happier, healthier life.
Participants receive:
- Education about the condition and the need to follow-through with treatment recommended by the physician
- Access to a nurse for specific questions and to discuss preventive and self-care strategies
- Health coaching related to self-management goals
- Invitations to SummaCare’s education seminars
