Summary of Responsibilities
Performs telephonic screening, interview documentation, mailings and reporting for health plan members.

Essential Functions
1. Completes telephonic health risk assessment for new enrollees and refers to RN Care Coordinator if immediate attention required.
2. Completes telephonic post-hospitalization health screening calls and notifies RN Care Coordinator of all findings.
3. Assists new enrollees, as needed, with scheduling or facilitating the scheduling of PCP and specialist appointments, and accessing specific clinical information to support enrollees needing case and disease management.
4. Documents attempts to contact members in the Health Risk Assessment (HRA) database.
5. Reviews reports and facilitates contacts (telephonic/mailing) to identify enrollees that are behind on preventative screenings as dictated by state and federal regulations.
6. Facilitates unable-to-contact and initial disease management mailings to enrollees.
7. Performs accurate data entry for assigned tasks including health risk appraisals, authorizations, discharges, etc.
8. Assists department in meeting timeliness standards including, but not limited to, data entry, member/provider mailings, and initial and follow-up telephonic health risk screenings.
9. Performs all job functions with integrity. Provides timely internal and external customer service in cooperative, professional, and respectful manner.

MINIMUM JOB REQUIREMENTS
Educational Requirements
:
High School Diploma/GED required.

Required Experience:
1+ years performing same or similar responsibilities. Relevant experience includes the following: combination of clerical and customer service experience in any industry with any or all of the following experience:
• Clerical support duties: data entry, typing, and/or filing of correspondence.
• Customer service experience: telephone customer service, call center experience, or/or experience working with the public in any industry.

Required Licenses and/or Certificates:
None required

Software and Data Entry Requirement:
1. Typing of 35 WPM with 96% accuracy rate.
