Condition Management: Understanding and managing your chronic condition is key to a healthier life

More than half of Americans, or 6 in 10 adults, are living with a chronic illness, according to the Centers for Disease Control and Prevention. A chronic condition is a long-lasting, incurable health issue that requires ongoing medical attention, such as diabetes, asthma, depression and heart disease.
People with chronic illnesses are more likely to be hospitalized and may struggle with work and performing daily tasks. 

The good news is understanding your chronic condition and learning how best to manage it are key to living a healthier and happier life. 

So whether you have one or more chronic conditions, are newly diagnosed or have a chronic condition you have lived with for years, SummaCare offers Condition Management programs that provide you with valuable tools, resources, health coaching and the most up-to-date information to help you manage your chronic condition. 

Our goal is to help you learn skills to manage your symptoms, reduce stress, and improve your health and functional ability.

What to expect from SummaCare’s Condition Management programs

Participation and activities in these voluntary programs are tailored to your disease severity and needs. Once enrolled, you can expect to receive the following: 

A letter explaining the program.
Educational information with advice on managing your condition.
Access to a SummaCare nurse to answer questions or provide health coaching support and materials.
Help finding a doctor, obtaining community services and care in the SummaCare network.
Follow-up after any hospitalization for your chronic condition to help with post-discharge needs for care or services.
Advice and reminders regarding preventive and self-care for your overall health.

If you need additional care services, you will be invited to participate in personalized health coaching where you will receive the following:

A condition assessment conducted over the telephone.
A personalized chronic condition self-management plan tailored to your goals and needs.
Easy-to-use educational materials specific to your self-management and treatment plan.
Ongoing telephone contact with a nurse to help coach you through accomplishing your health goals.
Condition-specific equipment for preventive care, such as a peak flow meter to help predict an asthma episode before symptoms appear or a remote telemonitoring system to detect heart rhythm and rate, and more.

You may also receive:

Access to the SummaCare WebMD® portal featuring an online library of chronic conditions and other health information, symptom tracking tools and educational videos about managing your condition.
A 24-hour phone number to speak with a nurse about your condition or other health questions.
