This Privacy Policy describes how SummaCare collects, uses, stores and protects information submitted through our Meta lead forms. By completing and submitting a lead form, you acknowledge and agree to the practices described in this policy.
When you complete a SummaCare lead form on Meta (Facebook or Instagram), we collect only the information you explicitly provide. We do not collect any technical or device-related data, such as IP addresses, browser information, device type or location data.

Your contact information and form responses are sent to Salesforce, our customer relationship management (CRM) platform, and made available to our sales team. Our sales representatives may contact you via phone or email to discuss health insurance options, answer questions or assist you with enrollment.
For lead forms associated with a SummaCare-hosted event, your information is additionally used to:
Event registration data is stored in SummaCare's internal database and is accessible only to authorized administrators and members of our sales team.
Certain lead forms may be used to gather information in order to send you health plan information to aid your shopping/decision-making process. This may include:
This information is only collected to either distribute digital copies of SummaCare resources via email or digital download or physical materials via postal mail.
SummaCare does not sell, rent or share your personal information with third parties for their own marketing purposes. Your data is used solely for the purposes described in this policy.
All information collected through lead forms is transmitted to and stored in Salesforce, a trusted enterprise CRM platform that employs industry-standard security measures. For event-based forms, data is also stored in SummaCare's internal database.
Access to your information is restricted to authorized SummaCare administrators and sales team members who need it to fulfill the purposes described in this policy. We take reasonable technical and organizational steps to protect your data against unauthorized access, disclosure or misuse.
SummaCare retains your information for as long as necessary to fulfill the purposes described in this policy, including follow-up communications, event management and applicable legal or regulatory requirements. If you wish to have your data deleted, please see Section 5 below.
You have the right to request access to, correction of or deletion of the personal information SummaCare has collected from you through a lead form.

This policy applies to information collected by SummaCare through Meta lead forms. It does not govern Meta's own data practices. SummaCare reserves the right to update this Privacy Policy at any time. Any changes will be reflected by an updated Effective Date at the top of this document. We encourage you to review this policy periodically.
