Care Management and Condition Management Programs provide nurse-guided assistance and resources to help you manage your behavioral health and live a healthier life. If you are taking medication for a behavioral health diagnosis, you can find the following information by logging into your Plan Central account and selecting My RX Info:
- Drug information, such as potential side effects, drug interactions, or alternatives
- Home delivery options, communication preferences
- Benefit highlights, prescription history and upcoming refills
Go to Plan Central on your computer or mobile device to register or sign in. First-time users will need Member ID, name and date of birth.
