Employee Referral Campaign Rules
Who is eligible to refer?
SummaCare employees, Summa Health employees and Summa Health volunteers who are not current SummaCare Medicare Advantage members are eligible for this campaign.
Per CMS guidelines and regulations, employees who provide direct patient care (physicians, physician assistants, nurse practitioners, nurses, pharmacists, etc.), cannot refer their own patients to SummaCare Medicare Advantage plans.
SummaCare Marketing, Sales and Member Services staff are also excluded from this campaign.
Who can be referred?
The person being referred must be Medicare eligible or eligible to enroll in Medicare within the next 12 months.
The person being referred cannot already be a SummaCare Medicare Advantage plan member.
The person being referred cannot be someone who is an active Summa Health employee, SummaCare employee or Summa Health volunteer.
The person being referred must be residing in one of the 33 Northern Ohio service area counties: Allen, Ashland, Ashtabula, Auglaize, Carroll, Columbiana, Cuyahoga, Defiance, Erie, Fulton, Geauga, Hancock, Henry, Holmes, Huron, Lake, Lorain, Lucas, Mahoning, Medina, Mercer, Ottawa, Portage, Putnam, Sandusky, Seneca, Stark, Summit, Trumbull, Tuscarawas, Van Wert, Wayne, and Wood counties.
The person being referred must not have previously contacted SummaCare for information within the last six months.
Other Campaign Rules
Receiving a promotional offer is not contingent on the referred person enrolling in a SummaCare Medicare Advantage plan.
The person being referred will have three ways to submit their information in order for the employee or volunteer referring them to be eligible for entry in the campaign:
The person being referred must submit their own information on the landing page summacare.com/refermedicare.
The person being referred must identify the employee as the person who referred them when they complete and submit the form on the landing page; or
The person being referred must call 800.292.7873 (TTY 711) to talk with a licensed Medicare Advisor and identify the referring employee or volunteer.
Eligible persons will receive promotional offers up to a maximum incentive of $50 per employee/volunteer.
Referral entries are on a first-come, first-serve basis. If the person being referred is submitted twice, the earliest submission will receive the gift card.
The employees or volunteers who participate in the campaign by referring a Medicare-eligible will receive their gift card(s) within 30 days of the referral date. They will be subject to all applicable taxes.
