Member Account

Secure, easy access to your plan and benefit information. Create a member account using your SummaCare Member ID number found on your Member ID card.

  • Request to receive electronic explanation of benefits (EOBs)
  • View and print explanation of benefits (EOB) 
  • Review benefit information including copays and any applicable deductibles
  • View the current status and history of eligibility, claims and authorizations
  • View amounts accumulated toward any applicable deductibles
  • Request a Member ID card and/or print a temporary Member ID card
  • View or edit Primary Care Provider information
  • View and print your schedule of benefits (SOBs)
  • View and update coordination of benefits (COBs) status

 

Provider Search

As a SummaCare member with a Preferred Provider Organization (PPO) plan, you have access to thousands of providers who are part of your SummaCare network. To find your provider or hospital, use SummaCare’s online provider search tool. You can do the following within the tool:

  • Find providers by name, address, county or specialty
  • Narrow your search to include gender, zip code, languages spoken and hospital affiliation
  • View whether or not the provider is accepting new patients or has extended office hours
