If you are enrolled in a SummaCare Medicare Advantage plan, and your plan covers out-of-network services, you may see any Medicare-approved provider.
If you are enrolled in a SummaCare Individual or Family plan, please note that you are only covered for emergency or urgent care services outside of the SummaCare four-county service area of Summit, Stark, Portage and Medina counties. To minimize your costs for emergency or urgent care services outside of this service area, please use the networks listed below.
To minimize your out-of-pocket costs when outside of the SummaCare 20-county service area, please refer to the back of your SummaCare Member ID card to determine the networks available to you.
