Following a hospital stay, or if you are diabetic with an A1C over 8, you can receive home-delivered meals to help you focus on rest and recovery. Mom's Meals is not available on the Topaz (HMO) or Quartz (HMO) plans.

Helping Members Recover and Manage Chronic Conditions
You can receive home-delivered, nutritious meals to help you focus on rest, recovery and managing chronic conditions.
Members on the Amber (HMO) plan can receive 28 meals (2 per day for 14 days) available to them post discharge from the hospital and/or for diabetes with high A1C of 8 or more.
Members on the Ruby (HMO), Sapphire (HMO-POS), Emerald (HMO-POS), Summa Health Retirees (HMO) or Summa Health Retirees (HMO-POS) can receive 14 meals (2 per day for 7 days) available to them post discharge from the hospital and/or for diabetes with a high A1C of 8 or more.
This benefit is not available on Quartz (HMO) or Topaz (HMO).
