Care and Condition Management Programs provide nurse-guided assistance and resources to help you manage your behavioral health and live a healthier life. If you are taking medication for a behavioral health diagnosis, you can find the following information under your personal prescription manager:
- Drug information, such as potential side effects, drug interactions, or alternatives
- Home delivery options, communication preferences
- Benefit highlights, prescription history and upcoming refills
