SummaCare’s Programs

Care and Condition Management programs provide nurse-guided assistance and resources to help you manage your behavioral health and live a healthier life. Depending on your needs, a registered nurse will help you navigate your healthcare and provide needed support. Call 877.888.1164 (TTY 711) for more information.

Care Management

When a member is diagnosed with complex health problems, a SummaCare Care Manager monitors the clinical care the member receives.

Care Managers are registered nurses who assist members by helping with transitions between hospital and home or other care settings and help members access needed health and support services. Also, a member of the team is a licensed social worker who can assist with finding community resources, transportation or provide support during illness, domestic conflicts or abuse.

Complex Care Management

Care management services may be long-term for members with catastrophic or highly complex problems. For members with complex needs, SummaCare Care Managers will assist in understanding the condition and will coordinate arrangements for complicated plans of care. The SummaCare Care Manager stays in touch with healthcare providers and other caregivers and provides assistance with finding needed care within the plan network.

Pediatric-to-Adult Care Management

A Care Manager is available to help SummaCare members arrange for adult care during a transition from child/adolescent needs to adulthood. Our provider search is also available to help find a new provider for someone entering adulthood. 

SummaCare’s Care Management programs are available at no cost to you. To learn more about or to enroll in SummaCare’s Care Management Programs, call 330.996.8931 or 877.888.1164 (TTY 711) 8:30 a.m. to 5:00 p.m. EST. Enrollment is optional and you may opt out at any time.

PregnancyCare

This program is available to high-risk expectant moms. Assistance is provided to support the health of the mother during pregnancy and right after the birth of the baby. Pregnancy and newborn care education is available as well as assistance with referrals to community resources. Enrollment is at no cost and confidential.

Short-Term Care Management

Care management services may be short-term for members who are transitioning home after a hospital stay or who are new to our plan and need help finding new in-network providers. 

Transitional Care Management

In the event that a SummaCare member reaches the plan limits on certain types of care, a SummaCare Care Manager is available to help the member find other resources.

24-Hour Nurse Line

Experienced registered nurses have accurate, up-to-date information about almost any health topic. A nurse can answer your questions, offer support and help you make decisions about any non-emergency health situation. All at no cost to you. Flu Vaccines

SummaCare members can receive the flu vaccination at no cost (no co-pay) from any in-network pharmacy. Click here to begin entering your search criteria. Remember to call the pharmacy in advance to make sure they offer vaccinations, have the vaccine available and the best time of day to get a vaccination. Simply present your SummaCare Member ID card and no co-pay will be required.

If you don’t want to go to an in-network pharmacy, check with your SummaCare provider to see if they have vaccines available. You may be asked for a co-pay if you receive additional services during your office visit. Please note: A visit to an urgent care center for your flu vaccine will result in an urgent care co-pay.

Health Coaching

Health Coaching

Health coaching is a one-on-one telephonic service offered to help you with lifestyle behavior modifications to manage/prevent future diagnosis of chronic conditions. Your health coach will help you identify at-risk behaviors and provide suggestions to improve your health and wellness. Learn more about Health Coaching.

Preventive Health Screenings & Reminders

Do you sometimes forget to schedule regular check-ups? We’ll send you reminders for preventive services and other recommended care for common conditions and preventive screenings.  

Preventive care is the most important step you can take to manage your health. Routine health exams and early detection can help prevent serious illness. The list of recommended guidelines for you and your family is below. Please talk with your healthcare provider to determine which of these are appropriate for you. 

Adults

  • Annual flu vaccine
  • Annual wellness visit with appropriate vaccinations, review of medications, BMI and blood pressure screening
  • Bone mineral density test for osteoporosis
  • Breast cancer screening/mammogram
  • Cervical cancer screening: HPV screening/Pap smear 
  • Colorectal cancer screening
  • Depression screening
  • Diabetes screening: A1C, retinal eye exam, kidney health evaluation, blood pressure screening
  • Lipid profile screening (cholesterol)
  • Lung cancer screening
  • Nutrition, physical activity and urinary incontinence counseling
  • One-time Hepatitis C screening
  • Smoking and tobacco prevention/cessation counseling

Infants

  • Well visits with appropriate vaccinations prior to two years of age

Children & Adolescents

  • Well visits with appropriate vaccinations such as HPV (Human Papillomavirus), meningococcal, and Tdap (tetanus, diphtheria, and acellular pertussis) and others
  • Annual flu vaccination
  • Smoking and tobacco prevention/cessation counseling
