SummaCare Provider Networks

We offer a variety of networks to ensure our members have access to the care they need. Your network depends on the type of plan you have, so always refer to your SummaCare Member ID Card to determine your network name.

To find your provider network, refer to the logo on the upper right corner of your SummaCare Member ID Card.

Individual & Family Plans (for people under age 65; purchased on- and off-Marketplace)

SCConnect Network
SCConnect is an exclusive network of physicians, facilities and hospitals including Summa Health System, Cleveland Clinic Mercy Hospital, Mercy Medical Center (Canton) and Akron Children’s Hospital. It’s available to residents of Summit, Stark, Portage and Medina counties and offers efficient care at lower costs. See a list of hospitals in the SCConnect Network.

Medicare Advantage Plans (for people over age 65)

SCMedicare Network
SCMedicare is an extensive HMO network specifically for our Medicare Advantage members. It includes more than 26,000 northern Ohio healthcare providers and 80 hospitals across 33 northern Ohio counties.
