SummaCare's Condition Management programs provide specialized services to members who have been diagnosed with conditions such as asthma, depression, diabetes, heart failure and high-risk pregnancy.

- SummaCare's 24-Hour Nurse Line where experienced registered nurses have accurate, up-to-date information about almost any health topic. A nurse can answer questions, offer support and help make decisions about any non-emergency health situation.
- Preventive Health Reminders for preventive services and other recommended care for common conditions and preventive screenings.
