Get to know our SummaCare vendors: partners in keeping you well

As a member of SummaCare you may or may not be called by one of our vendors below to offer you a service or information on behalf of SummaCare. If you have questions, please call the Customer Service number on the back of your member ID card.

Cooperative Health Partners (CHP)

  • Assists members with non-911 medical transportation/ambulance needs.

EMMI

  • Provides reminders to schedule preventive screenings, including phone calls.

EpiSource

  • Providers in-home health assessments for Medicare and non-pediatric ACA members

eviCore

  • Provides assistance in selecting HiTech Radiology and Oncology services.

MedImpact

  • Provides members reminder calls to refill their prescription if they are more than seven days late.

Medtronic Care Management Services

  • Offers in-home remote patient monitoring for select members with congestive heart failure (CHF), coronary artery disease (CAD), chronic obstructive pulmonary disease (COPD), hypertension and/or diabetes.

Everly Health (formerly Home Access)

  • Provides in-home tests, such as colorectal cancer screening FIT (fecal immunochemical test) kits, to select members.

Homelink

  • Offers members durable medical equipment (DME) supplies and equipment, including preferred diabetes testing supplies, for all SummaCare members per benefit. Including the following Medicare plan benefits: hearing aids, home safety devices, non-medical transportation/ride-share, and massage therapy/acupuncture.

National Jewish Health

  • Offers telephonic smoking cessation counseling and nicotine replacement therapy to members, referred to as SummaCare’s QuitCare program.

Papa

  • Papa is a company that pairs Medicare beneficiaries with adults, called Papa Pals, for companionship, technology education and support, assistance with everyday tasks and more.

Quest HealthConnect (formerly MedXM)

  • Offers eligible members in-home heel bone density scans and diabetes retinal eye exams.

Summa Health System Clinical Access Center

  • Provides the SummaCare 24-Hour Nurse Line service and also assists SummaCare's Quality Management team to schedule mammograms, prescription refill reminders or to promote SummaCare's programs.

SummaCare OTC

  • Offers Medicare members over-the-counter (OTC) items (select list of OTC first aid type items).

Symphony Performance Health Analytics (SPH Analytics)

  • Administers surveys to our members, including those required by CMS. These surveys help SummaCare evaluate and improve services and programs.

Turn-Key/CareCentrix

  • Provides specialized team-based care focused on relieving the symptoms and stress of a serious illness.

WebMD

  • Provides members a personalized health and wellness portal to help track healthy lifestyle habits.
